Care Management Specialist - qualified applicants will hold a current unrestricted Florida RN or LPN state Licensure and graduated from an accredited RN/Associates Degree or LPN program. Five years clinical practice required, knowledge of Microsoft Office products is required, including intermediate to advanced Excel and Outlook skills. Assumes the responsibility for care coordination and related case management, makes the appropriate referrals to utilization management and disease management activities within Healthchoice. The ability to communicate both verbally and in written format medical information regarding patient assessments, and follows up with patients and the appropriate providers regularly to ensure patient compliance. Navigate and document within the Care Management system, review findings, patient related assessment and actions and identify solutions to non-standard requests and problems. Assesses barriers to treatment goals and assists patients with removing those barriers. Communicates with the patients and the physicians between provider visits verbally or telephonically as needed. Assists in the coordination of any additional needed outpatient care. Coordinates care within the managed care network or when necessary is able to identify the appropriate cost effective care for health plan members within the community if indicated.109660
